Wandering Eye
Wondrous Item, Legendary (Requires Attunement)
The Wandering Eye is a thorn-covered ocular artifact of extraordinary divinatory power. At a glance, it appears as a severed eye encased in brambles, thorns, and strange organic growth. Despite its grotesque form, the eye is not inert. It is aware enough to communicate telepathically, or else serves as a conduit for something that can.
The artifact first came into the possession of Venathrax during the expedition into the Dreadmist Swamp in search of the sad tree seen in the drawings of Agrovionimb’s children. During the night, Venathrax awoke to find the Wandering Eye in his pocket. The object radiated powerful divination magic, and a voice spoke directly into his mind, claiming that the eye was a gift meant to aid him on the dangerous path ahead.
When Corq examined the item with Identify, he recognized it as a legendary artifact and immediately advised Venathrax to get rid of it. Anchor, by contrast, viewed the eye as a powerful enhancement and encouraged its use.
Against Corq’s advice, Venathrax removed one of his own eyes and placed the Wandering Eye into the empty socket. The artifact immediately grafted itself into place. After Wrenn healed the wound, Venathrax’s vision was permanently changed.
While attuned to the Wandering Eye, Venathrax possesses truesight out to 240 feet.

Properties¶
While attuned to the Wandering Eye, a creature gains Truesight to a range of 240 feet.
Attunement¶
To attune to the Wandering Eye, a creature must remove one of its eyes and place the Wandering Eye within the empty socket.
The eye immediately grafts itself into place.
Removal¶
The Wandering Eye may be removed during a short rest, ending attunement.
Removing the eye leaves the bearer blind in that eye until it is restored by magic.
Known Bearers¶
Venathrax¶
Venathrax became the known bearer of the Wandering Eye on the night of the 16th of Firemoot, 1137 AoD, while camped in the Dreadmist Swamp.
The procedure was performed in the field during the party’s expedition. Corq strongly objected. Anchor supported the decision. Wrenn provided healing after the eye was implanted.
No one in the camp slept particularly well afterward.
Unanswered Questions¶
The Wandering Eye’s origin remains unknown.
Its arrival was not accidental. The eye appeared in Venathrax’s possession without explanation, spoke into his mind, and urged him to accept it. The voice presented the artifact as a gift, but neither Venathrax nor the party knows who truly sent it, what purpose it serves, or what cost may yet remain unpaid.
What is known is that the Wandering Eye is powerful, invasive, and almost certainly not a simple gift.
